This purpose of this Privacy Policy is to describe the collection, use and retention practices that apply to your use of the Driving & Dementia Roadmap (“DDR”) website (the “Website”). In this Privacy Policy, the terms “we”, “us”, and “our” refers to Baycrest Centre for Geriatric Care (“Baycrest”) and Sunnybrook Research Institute (“SRI”), the owners and/or operators of the Website.

The Website is not a data repository or electronic health record and may not be used to store personal health information under any circumstances, nor is the Website intended as a substitute for maintenance of proper case reports by a qualified healthcare professional in accordance with applicable clinical and organizational guidelines.

With the exception of the Automatic Information collected as defined below, we do not collect information that would personally identify you unless you choose to provide it to us.

Automatic Information Collection

The Website operates using Google Analytics which automatically collects information as part of the standard operating process of the Website. The information collected includes your internet protocol (IP) address, cookie information and browsing actions and patterns such as user analytics.
